Lost Your COVID-19 Vaccine Card?

You lost your COVID vaccine card and now you need to show proof of vaccination. What do you do? Well, there's no need to panic.

There are a couple of different ways to get a replacement card, as well as to verify your vaccination status. Start off by contacting the place where you got the vaccine. That's likely a pharmacy, a doctor's office, or maybe your city or state health department.

Every place that administers the COVID vaccine keeps records of who received, which shot, and when. They should be able to make and send you a replacement card. Many of these places though also have an online verification system, where you can put in your information and request a new card, or usually download one electronically, and then you can print a new one.

Some states are also creating personal QR codes, which contain your vaccination status, but keeps your personal information, such as your date of birth, private. Do not try to contact the CDC, because they do not keep your personal vaccination record. Don't even think about falsifying a vaccine card, because doing so is a crime and that could land you in jail.

I know some folks have been laminating the cards, but with the need for boosters, please don't do that. It makes it very hard to update the card. And by now, you likely realize you need to take a picture of the vaccine card with your smartphone. Now, some people are putting it in photos. Other people are scanning it and putting it in the note icon, so it's readily available.
